Therapy Portal

TBT now uses an online platform, Therapy Notes, to collect all client intake information. This online platform allows clients to create an account and complete paperwork securely and easily. Please contact us to be sent a unique login link to create an account and access your specific intake forms. If you have already created an account with Therapy Notes, you can log back in by clicking this link below:

If you need help completing these forms online, please email our admin team (info@tetonbehaviortherapy.com) for a PDF copy to print and fill out at home. Please remember to bring this paperwork with you to your first scheduled appointment. If you cannot print at home, please arrive 15 minutes before your first scheduled appointment to complete the intake paperwork in our office (remember to bring your insurance and a credit card).

Client rights and responsibilities are important to us at Teton Behavior Therapy. Please find a link to our HIPAA and PHI privacy agreement below. Copies of these forms are found in your initial intake packet, signed and dated by clients and clinicians.

Fees

We strive to work with every family to make sure that finances do not stand in the way of getting them the help they are looking for.

Teton Behavior Therapy is an in-network provider for the following:  

  • Blue Cross Blue Shield Wyoming
  • Blue Cross of Idaho
  • Wyoming Medicaid 
  • Mountain Health Co-Op
  • Regence Blue Shield of Idaho  
  • UnitedHealthcare 
  • Idaho/Wyoming Medicare Part B (J3-Noridain) 

We accept payments from: 

  • ID Juvenile Probation
  • ID Children’s Mental Health
  • Wyoming Department of Family Services
  • Crime Victims Compensation

Cancellation Policy:

At Teton Behavior Therapy, we understand that life can be unpredictable, and sometimes, you may need to cancel an appointment. We kindly ask for 24 hours’ notice to help us accommodate other clients in need of support. If you need to cancel with less than 24 hours’ notice, you will be charged a $150 cancellation fee. This policy helps us maintain consistent care and ensures that we can serve everyone as effectively as possible.

We are committed to helping you manage and maintain your mental health needs. When you schedule an appointment time with one of our Therapists, a time is reserved for you, and your therapist will be preparing to help support you in your therapeutic goals. We understand that on occasion unforeseen circumstances arise and the need to cancel your scheduled appointment may be necessary.

No-Show and late-cancellations of your appointment cause problems that go beyond staffing and financial impacts on your therapist and the practice. When an appointment is missed or canceled less than 24 hours prior, it takes an available time slot away from another client, delaying the delivery of care to someone who may be in urgent need. 

If you know that you will be unable to keep your appointment, we ask that you notify us and/or your clinician to cancel; 307-734-6040, press 1.

Reminder emails or texts will be sent as a courtesy prior to your appointment. Patients are then responsible to remember to their scheduled appointment or reach out to cancel 24 hours prior.  

*Those clients utilizing WY Medicaid or working with one of our master’s level therapist interns will have a $25 late-cancellation fee applied in these situations. Due to our contract with ID Medicaid, we are not permitted to bill clients for late cancellations or missed session fees. Please be mindful of the impact these situations have on the financial needs of your individual therapist.
